I had this exact same issue. Support said they hold deleted leads in a "soft delete" for 30 days, and after that the change will be reflected in your segmentations.
Are you saying you would like to launch an email campaign to your unsubscribed leads to try to get them to opt back in? That is illegal. You can't email them. They unsubscribed.

You could run a report of leads that have no Lifecycle Status or Lead Score.
Hmm. I do have a campaign that logs an MQL Date, but it's triggered off of the Lifecycle Status changing to MQL.
