There isn't a report that will show you all of that in a tabular form. However, you can get that information by running a series of smart lists. You can also add a constraint on the number of times a form has been filled out.
Is you are looking at the Email Performance report, it should tell you what time zone it is using. If it shows EST, then, that is what it is using. You can check quickly by finding someone who was sent the email and compare the time of the activity log with what you see in the report.

If you are talking about report subscriptions, they run at night. I normally receive them around 3 - 4 am ET.

I would first find out how big is the problem. Go to the System Smart Lists in the Lead Database and look for the Possible Duplicates list.
