I believe the attachment can still be sent, but when the email is logged in Sales Insight, only the body of the email will be visible.
You can bring in the filter in the web page report's smart list: "Visited web page: in past 30 days". Also in the "setup" change the date of activity to the last 30 days as well.
In the form editor when you click on the field, do you see the "field type"? You can select radio buttons from there and then click "values" towards the bottom to input those you would like.
Are these prospects not already in Marketo? If not, you will need to import them as "leads" otherwise Marketo cannot email them.
As far as I know, this is not possible because Marketo logs the email activity to the lead or contact itself.
Choose whichever "Name" you would like the field to appear as in Marketo. The "API name" will autogenerate and is used in the backend, so that name won't appear in Marketo.
