Good to know Dan. Are you aware if there is a limit to how many active engagement programs you can have or a limit to how many you can have in general?
In addition to what Dory suggested, a couple times when I came across this issue I was already using the field in the form so the field name was not available on the drop down list to add a new field.
There is no limit to how many engagement programs you can have.
An engagement program can have a maximum of 15 streams. I am not aware of a limit to how many emails can be in any one stream but I have streams with 20 emails in them.

There is a system notification for campaign failures: Notification Types - Marketo Docs - Product Docs. From there you can subscribe to that notification type and receive an email alert in addition to the system notification.
