Did the record's activity log record a Merge activity? When two records are merged in Salesforce Marketo will sum their scores together so this really looks like a merge rather than a conversion...
Unfortunately that's too far back; the data and the record would have been totally purged from Marketo. Support can (usually) investigate deleted records within a week of deletion before the underlying data is lost.
If it's been deleted very recently then Marketo Support can tell you the cause of the deletion.

Your first cast is set to a past date. The red highlight on that field is saying the value is invalid. Set the initial cast for the new schedule to be a date in the future and it should save.

Change your List Import mode to skip new people and updates. The list will just populate with people already in your database.
You can't add or delete any fields from the Check-In App. If you need a custom form you'll have to host it on a landing page and display that on the tablet rather than use the app.
Only one Delivered (or any enabled synced activity) event will sync per person, per day. So if someone is delivered two emails in the same day, only the first one will sync. Do they lead/contact you're checking have more than one delivered email in the same day?
