As Josh mentioned. I would create hidden fields on the form that capture the parameters such as utm_source utm_campaign etc and then trigger off of values in those fields.

I would add to the checklist that if you have EU or APAC countries to save the file as UTF-8 format or you will have funky characters. That kills me everytime.

Many data appending vendors will append the industry data for you such as ReachForce.
