Ashley, Wintha Kelati is absolutely right. I thought you were using an external campaign for your transitions, in which case this would be an odd behavior.
Vivian, also check that you are emailing the email address she uses to unsubscribe. Let's say you are emailing her @domain1.com but she is unsubscribing from @domain2.com - I have seen that happen more than once.
Nathan, there is no out-of-the box report that will tell you that, but if you have set your progression statuses to FILLED OUT FORM or similar, then you could create a program performance report, filter by program (and check the option to show progression statuses).
Karen, this is a feature of the Revenue Cycle Analytics model. If you do not have it, you will not have access to this type of report.

You could ask Marketo support to slow the sync. I think the default is 5 min or so - you could slow it to 15. It would create a bigger window for people to update the records but it would not be dramatically laggy.
Hi Gregory, Look into the lead assignment rules. If you are using the SFDC default rules and you assign leads to a queue, that could cause duplicates.
