That would indeed be where I'd start my investigation. Do I see the activities in the log, when I run a smart list on people having an opportunity meeting the criteria, do any people qualify?
If there are no activities shown and no people qualifying for the smart campaign I would suggest to have a look at your smart list. Seems there is something wrong with your triggers / filters. To validate whether the sync actually works you could indeed try to sync one of those leads manually.

That's absolutely right, @Darshil_Shah1 ! It is not really a matter of an email being 'vulnerable' to 'attacks'. These are not attacks, quite the contrary,. They are a protective measure on the recipient's end. I do see that Marketo are taking steps towards handling these activities in a better way ...
Did you by any chance send a newsletter to that 98k audience? In that case you can filter your smart list on people who were sent that email and are no longer part of your subscriber base. That might help narrow down what happened to them in the activity log.

I have to disagree with the concept of using a nurture program for this flow to start with. The Default program you describe will do the job for you, so there is no need at all to build a whole nurture around this.
You need to set your Default choice to Do Nothing (remove the value that is in there currently), otherwise everyone will be removed from the flow.After that last flow step, you do still need to send the email to all people who are left in the flow, so a simple send email without any further choices.
