Try creating the smart list in the leads database or marketing activities.
I just tested this and I'm able to see my form on the smart list. Is your form approved in Design Studio? If it's not approved maybe that's why you're not seeing it?
How about creating a smart list filled out form (name of your form) add if the form is used in more than one landing page, add constraint (web page). You should be able to see who filled out the form.
I'm on Chrome on a Mac and it's working fine for me.
I've used Grazitti and I'm really pleased with their work. We had a freelance graphic designer do the mock up for the email and we had Grazitti do the code work. Most of the email templates that are on templates.marketo.com are designed by Grazitti, which I've used and tweaked.
I'm not a fan of the arrows that move the text copy up and down, it's so cumbersome and the spacing often looks funny and takes me a while just to edit. If there's just that one box that allows you to copy and paste text, it would be much easier.
