Isn't the problem that the domain doesn't match the product? IE the domain can't be applied regardless of what precedes it?
Not for a single domain, but for multiple you have to talk to support since out of the box you get one "______.domain.com" for landing pages, unless I didn't understand the question.

How many times can leads flow through the campaign? You can set it for "once every 365 days" which should work as long as your qualification/triggers are set up properly.
I doubt they're going to let us crack the code on this one, but you can ask support and they typically know what you're on.
