Did you mean, "Will other cnames be impacted?". I don't believe so. I've seen IT teams create a cname where they only work externally and don't work internally so I'm guessing they can do the opposite.

I've seen some creative SFDC Admins implement a solution to update the systemmodstamp whenever the lookup field is updated so the value automatically syncs to Marketo.
Whenever creating a new field you are planning to sync to Marketo, make sure you make the field visible to the Marketo salesforce profile. The information provided by Robert about timing is correct. Thanks!
