Nothing jumps out to me that would be causing the conflict. You could either pull out each script one by one to narrow down where the problem is, or you could consider placing the form in an iframe instead, which would eliminate the conflicts.
Are you getting any sort of error message? Will it not approve the email program, or it approves and doesn't send at the time you gave?
Have you tried using the advanced editor to see if it works then? This sounds like a bug more than anything else--I would recommend opening a support ticket.

You can't use the champion/challenger in an engagement stream PROGRAM. It will work only if you add the email directly to the stream. Vote for that to be fixed here: https://community.marketo.com/MarketoIdeaDetail?id=08750000000JwKxAAK
Also, it's important to note that if you use any sort of segmentation within your email, you CAN'T use email scripts (thus my method wouldn't work).
