What you might want to consider is using an iFrame approach for each form. This way the form wouldn't appear again after submitting it once and you also wouldn't have to worry about the entire page reloading each time they submit.

You can use tokens in change data value flows. So instead of giving a number, you can put {{lead.Lead Score}} or whatever the field is you want to copy.
