Not sure you can do this in a way that would allow you to actually see a list of Forms. You could constrain a report by Form IS ANY in Past X Days, but that won't show you want you want.
I agree wtih Jeff.Someone could use Preview with or w/o images on, so this is not reliable or helpful. You want engagement like CTR.

Can you research more to see if there are log errors?It sounds like the marketo user isn't able to do something. Perhaps Support can check the hidden logs or you can walk through the Marketo User permissions in more detail. It could be as simple as a field issue or a R/W permission.
You can put it together using formula fields based on the Account's contacts' scores. But you may end up doing that inside SFDC.
