Are you adding the same form as the pop-up?I've put two forms on a page before, but they were two different forms and not the same form twice.
One way around this would be to trigger the sending of the confirmation email upon Status change, which is what I've been doing for this reason.
That would make sense - I haven't updated the integration in at least that long, so it was most likely a timeout (and of course it happened after I'd told everyone I've never had it happen to me )
