    Dan, do people see a Thank You page after the form has been filled out, that they couldn't see elsewhere? You could report on views of the thank you page assuming that they can only get that from filling out the form.

    Mariola, did you create a campaign to track the form activity, or is it on a Marketo Landing Page? In case you didn't, like Tiff said, you can create a smart list of form completions which should give you that data.
