Yes, I fixed the syntax error before I tested originally, which still didn't trigger an event on submit. I'll keep researching before I start a new thread. Thanks for your help thus far. I have tag manager working on other aspects of the page, like scroll depth etc. so I know some is working/loading...
First, I'll focus on the code as written above "did not trigger an event in GA" when form was submitted. Has it worked, as written in original post, for others?

I used MktoForms2 :: gdubrow/Marketo Form Submission , from below. I have 2 forms on the page, different ID's When the 'test-form' is submitted I get duplicate records created. Both forms DO collect emails, but use different IDs
What exactly was the solution to the multiple records/submissions made in the database?
