It'll capture only new leads created. if you want to capture changes for email field, you can add "Data value changes" trigger (Attribute = Email address)
We import those leads also. Usually our SDR team complete the email address from other systems (Linkedin, Leadspace etc...).I think the only problem that can occur is when you have validation rule on "Email missing" in SFDC.

We implemented something very similar (based on this useful document: https://community.marketo.com/MarketoArticle?id=kA050000000KzMuCAK). Amber's suggestion is how we implemented this.
If the leads you added are fit to the smart list filters, then they will be added automatically to this smart list.
