Suzanne, I need the field updated in Marketo because I want to see how many accounts have opportunities in a specific stage (Roll Up Summary field can count opps based on specific criteria). This way I can email the contacts related to those accounts.

One solution is exporting all leads into an excel and filtering only the ones that "First Name = Last Name" , then create a custom field in Marketo. Create a column in the excel for this field (it could say "duplicate = yes") and import the excel back to Marketo (it will de-dupe based on the email)....
