Take a look at the activity logs to see what happened and why they got suspended. Is there a Marketing Suspended Cause? Did the emails all bounce? What type of activity preceded the suspension?
Do you have the view as webpage token baked into the email template?
The screenshots don't really demonstrate how many different leads were run through. Are these different leads that you're running through or the same lead over and over again?If these are different leads have you tried refreshing the count by clicking on the circular arrow?
From your screenshot, you only had the 3 fields as part of your progressive profiling. The first 4 fields "always show". Therefore, you'll see 7 fields altogether. 4 Fields that always show, and 3 progressive profiling fields.
During your cookied visit, did your lead record already have an Address, Zip Code, and Phone Number? If so, it's working correctly. If not, it should've shown you those 3 fields, or at least any field that did not already have a value.
Try using "Created At" is before "Put the start date of your form going live here".
Blocked leads are people in the Marketo database that are marked blacklisted, unsubscribed, marketing suspended, and that have invalid/empty email.
