Haven't seen this before. Try adding as the first line of code on your landing page template to see if that will help alleviate Marketo trying to append it.
Why don't you just do it on the SFDC side as a custom field and let that custom field sync down to Marketo. So basically, all the operations/formulas happen in SFDC and the field value then simply just syncs over to Marketo. Any particular reason why it has to be a Marketo only field?

From your logic, the date of when a lead hit 50 points is not an important factor in this because all you're really interested in knowing is when they hit 100 points, which is +50 additional points when they first hit 50 points, went into MQL and sales was notified. Thoughts?

Mainly because your email is in a regular program, then that program is added to an engagement program. If you added the email directly to the engagement program, it will work as expected.
I would recommend that you send yourself the email. Treat yourself a lead and do a single flow. When you receive the email, click on every single link and then go back to your lead to see the activity logs and see if that link is ever recorded. If you still can't find it, reach out to support.

Not with the out of the box analytics. You can get this with RCE or with building a smart lists for each lead source. If a visitor filled out a form or visited a page more than once, you'll need to build more smart lists to with minimum constraints to dive in even deeper to find the real numbers. ...
