Yes, this can be a possible case, have you made the settings of the campaign to be "Lead to Flow Everytime"? Because in this case a lead gets added to the program again even if you have removed them from flow.
Have you tried turning the program status "off" in the Setup Tab.
As Sanford Whiteman said, they are synced by default, so you just need to pick Email Opt Out field and add it to the report.

You just have to go to profiles section in salesforce and click on the user you have created and remove read access and edit access
