Isn't it easier to just disable form pre-fill for some (or all) of the form by going into the Form Editor and selecting the fields you'd like to disable and choosing "Form-Prefill = Disabled"
In the forms editor, go to "Form Setting" then click the little gear icon to "Edit Custom CSS".mktoForm .mktoLabel { float: right;}You'll probably need to add other CSS to make sure the padding and formatting is good.
Bob,You can also just select "New Email Template" to create an entirely new Email Template without importing anything. The only real requirement for an Email Template is that you have at least one "mktEditable"

That probably won't look good on different devices. Absolute positioning is not generally the best way to implement things.
