I'd love a new report type that shows form usage. Number of times filled out in a certain time period, last time used, etc.
Audrey, I would set up a support ticket for this issue. I can't think of anything and nobody has replied to this in over a day. I would setup a support ticket. Seems like an instance error.
FYI, you can set the instance default in admin settings under "smart campaign". See screenshot below.
Mary Kate, If you're using a smart campaign there is an option to have a max number of contacts the smart campaign hit (see screenshot).By default, it uses the instances default number. But you can change this for each individual campaign.

Your logic seems fine to me. Though, aren't they only getting the email if they fill out the form on the specific page? If so, I don't think there's a need to have the "filter: filled out the form on a specific page" in there.
