I would suspect that the lead's clock would restart because the lead did not flow through the entire campaign the first time.Try testing it with a 1 day campaign and remove the lead after an hour or 1 minute to see if it works.

Marketo usually syncs records that are synced (in both SFDC and Marketo) every 5 minutes. I know that is not completely true because I and others have had it take longer. I suspect Opps are lower on the priority list even if the Lead exists in both systems.

Did you refresh the smart list on its own? Sometimes they don't repopulate. But Marketo will always run it again before sending a campaign.

You can also use the filter Was Sent Email X so you send a different email instead during the flow step. Or use NOT Was Sent Email Y and then Send Email Y.I agree that Michael's Static List solution may be more elegant if you have a lot of content among seperate campaigns.
