Joanna has that right. It's possible for duplicates to not qualify for the same campaign if their attributes (other than email) are different.Email is only used to dedupe, otherwise the ID is the database key.

What are the other criteria for that smart list?Check your lead's activity log to see if something else is going on.
Updated in what way?Try Updated At On or After Today... but that won't be a trigger, so you really need to get more specific. Is the lead updated by a Form? By something else?
