Hi Chelsea, if you create the SFDC campaign in SFDC and then add people to the campaign via flow steps, you can have a SFDC campaign associated with more than one Marketo program. It sounds like this is what you're asking for, but let me know if it isn't.

Hi Michael, pick someone who was added to the program who you think shouldn't have been, then look in their activity log. You should be able to figure out their action or your smart campaign that added them.
Hey Sarah, if you create programs for all of the events you want to track and then add members and back-date their success, you should be able to get some degree of pre-existing activity. Just make sure you manually update their success date.
This seems incredibly targeted. Short of doing custom research, you might look at specific medical marketing publications for this type of information.
