Hey Liz,You can't embed a form, but you could have multiple buttons or link CTA's, and depending on which they click, could trigger the update in marketo.

Hi Loc, this isn't necessarily an alert, but you could set smart campaign restrictions in admin > smart campaign. And only remove this if you are running database-wide updates etc....
