You can also see information on everyone that has filled out the form on the campaign or create a smart list to see who has filled out the forms. On that smart list, you can create a custom view to see all of the fields they filled out on the form.
Have you refreshed your schema? We have found sometimes to see a new SFDC field, you have to pause the sync, refresh the schema under Admin Salesforce object sync and then turn the sync back on. Make sure you turn the sync back on once these steps are completed if you have to go that way.

...I just look at the last sync in the integration area in admin to make sure it is syncing, as well as looking at notifications of any sync errors that occur in the notifications area at the top of my user interface in Marketo.
If they have not been through all of the content, you could pause them when they are passed to sales, then change status back to active when they "reenter" the engagement program. I don't think they can go back through the same content within the same engagement program.

In order to get each person for each of those criteria, I would create smart lists related to finding those people. I don't know of a report that is going to show email metrics and show the people that fit those metrics. You can look at program members where it shows people with their program status...
I agree with Amit. 135/150 smart lists will really slow things down. Either of his suggestions should preform much better.
Could you try answering the surveys with test email addresses? If using a gmail address you can add info after the user name before the @. For example username+test061419a@gmail.com, username+test061419b@gmail.com. This would allow you to see results under each unique email address and then you can ...
