"Block field updates" only affects UPDATES to a field that already contains a value. If you select all, a value will still be able to be populated initially from any source.
Looks like you don't have permission to edit forms. You'll need to ask the admin who manages your Marketo instance.
A couple of reasons:There were some duplicate names in your listThere were existing records already in Marketo and Marketo deduped against those existing records (instead of creating new/duplicate records)
