You could build a form and then upon submit run a smart campaign that sends an email to the email address submitted on the form and then an alert email that gets sent to the sales person.
Mine seems to expand the folder structure just fine. However I'm only using one workspace.

I would edit one of the duplicate records email addresses to match the other. Then I would go to the possible duplicates system smart list, sort ascending, find the two dupes, select them both and click the merge leads button.
