Create an alert email that looks something like this (with the appropriate tokens based on your form).Create a smart campaign that triggers off of someone filling out a form, create a flow step that says 'send alert' and use the email you create. I suggest making the email operational as well.

Thanks Elliott, I got that piece I think... should have mentioned it. So do you run a triggered smart campaingn in a non-synced program that says if a program member is synced to SFDC to sync them to the appropriate SFDC campaign? I just want to make sure I'm automating it. Am I making sense? 🙂

Sorry Michelle, I mis-spoke (mis-typed). We use yes/no on the form but it's a boolean field. In SFDC they display as checkboxes. checked = true as in YES I want those. We use the fields on all email send smart lists. So if we're sending an event email a filter is 'Preferences - Events' = TRUEWe auto...
