Email address is the primary key within Marketo.No deduplication is needed via forms.There are other processes, such as manual lead creation, uploads/imports, and syncs from other systems that contain multiple records that can cause duplicates.
Just throwing it out there....What about you Channel Status setup and values, users cannot move backwards in the stages.Could the order or values be causing any issue?

Not using an average or median of the percentages as provided by Marketo.You must use the RAW numbers as I mentioned above (totals)
You need the actual # Delivered & Opened.From there you can calculate based on the total sum & then your percentage.
