Trigger campaign based on unique date

We are wanting to set up a trigger campaign to send an email to a client contact in our system based on their contract renewal date. Is there a way to set up this workflow in Marketo? Any tips are welcome!

Hi Jamie - thanks for your reply. We do use those embed codes on our LPs. Is it still possible to use the same form over and over again now that we aren't using Marketo LPs?
