I'm not seeing the custom date after the dash so I guess my custom date field isn't set up correctly.
OK, I added my tokens in and created a Lead Performance Report, but when I click on the Report tab I am getting an 'Error Running Report' message.

I have the same question as Loic. Our business team is requesting to know the exact dates that someone filled out a particular form on a particular page. The Smart List method doesn't seem like the right way to go because you'd have to keep changing your date range for each day. I saw your response ...

I think I am having the same issue.My landing page is approved and my progressive form is approved. I put the form on the page and when I tested it the first time I see my standard fields and 2 of the progressive fields (I used 2 for the number of blank fields) I hit submit and everything is working...
