Ryley, Have one of the options be "all of the above". Then have a smart campaign in the background check all those boxes on the contact's record if they choose the "select all option". Hope that helps!
Another option would be the number that it filtered out before the send (those that were marketing suspended/unsubscribed). Jarom you're going to want to keep the existing form until you've finished the new form. You should be able to edit the settings but it can be tricky. You can message me if you have questions.

Ranae, I would pull up the list you are wanting to change, select all, change data value, and change marketing suspended to "yes". Totally can make a smart campaign for this, but in my opinion, no need to create one for a simple task like this.
