You can use sandbox if your CRM supports that.Otherwise, just turn it on. There's no dry run with the live system.
You should use Forms 2.0 which supercedes the document mentioned.
You should use T/F in Marketo if that's what is showing. Marketo also display T/F as Y/N on the Form creation too.

I assume that if someone is already Unsubscribed=TRUE, that you would not set them to opted in under Opt In Express Conditions.I would consider rolling out your system to everyone when it makes sense to do so.
