sounds like both are issues of conflict with your global (or landing page) CSS.Try making a normal form with default settings and place that into an Iframe and check to see if it gets altered. If the same thing happens ist your CSSIf not altered, then its with the Form 2.0 settings.

That field may not have mapped correctly to Marketo. Marketo tends to show 'collected' data.
No unique per say but, because you have duplicate email addresses, Marketo is trying to find the record that is most recent to add the interesting moment to. Do you have all the MSI areas visible in your SFDC sandbox? Do you have many Interesting moments set up in your Marketo sandbox to fire off?
