The term "Account" here doesn't refer to what Salesforce calls an Account. This is referring to the company name - so it will pull in all SFDC Contacts and SFDC Leads with that company name into the "Account." The original lead existed in Salesforce and not in Marketo which is why Marketo didn't recognize the duplicate and pushed the new lead over the Salesforce.

Hi Laura,Create a Smart Campaign with a filter for "Filled Out Form." On the Schedule tab, click the number of leads associated and edit your view to contain the fields that are on the form. From there, you can export an Excel spreadsheet.
One of those leads must exist in SFDC and not Marketo. Since there is only one lead with that email address in Marketo, Marketo did not make a dupe record.
