My understanding is you would want to create custom fields on Program Members using your Standard user, if so you would need to ask your Marketo admin to get you Field Management access on your user roles.

I doubt if that is the case. You can request Marketo to get you a full copy sandbox Marketo instance and you can connect it with you MSD CRM (dev/sandbox version)

My best guess, your smart list did not qualified leads by the time you scheduled. Answer to you another question, Yes, flow step works for both batch and trigger campaigns
