We have a number of Smart Campaigns that are a child of a Program (a.k.a. nested in the Program folder). Some of the Programs have multiple Smart Campaigns and others only have on Smart Campaign.
When I look at the number of members in the Program it does not always equal the number of members in the Smart Campaign, nor is always the sum of members in Smart Campaigns. Oftentimes, the Program has 0 members while its child Campaigns have Members.
1. What is the relationship between Program Membership and child Campaign Membership?
2. If the Member is added to a Program through a Smart Campaign, does the Campaign Member need to specifically have a Program Status set for it to be considered a member of the Program (and if so why doesn't it default to some program status similar to how it would if it was captured through a form or list import)?
3. Does moving a Smart Campaign from one Program to another affect Program and Smart Campaign membership counts? What will happen to the historical counts? To be specific, what happens to the count of members already on the Campaign: is the count reset to zero, or does it remain at what it was and then any additional members while it is under the new program accumulate on top of the pre-existing count?
4. Are there any other changes similar to these that could be affecting the counts of Program Membership and Campaign Membership?

Agreed with all of the above. Program membership is used for reporting, understanding around a given tactic how far did people progress (via progression statuses). In a webinar, did they register, attend, no show etc. Smart campaign perform actions and do stuff which are separate from Programs and membership. This is more operational and I rarely use smart campaign members.

Effectively there isn't a relationship and you shouldn't expect to see a correlation between the two.
A smart campaign can qualify absolutely anyone in the lead database, or it can restrict it's qualifications to people who are members of the program. Likewise, running through a smart campaign does not automatically set membership of the program to true - this is only done in the flow with a "change status" flow step.
Alternatively, another smart campaign can restrict qualification to anyone who is a member of a different smart campaign, and so on.
