I have a generic form that can be used on multiple pages. However, I want to know from which page the user has submitted the request.
Kindly suggest how can I do this.

This has been asked many, many times.
On Marketo LPs, constrain by Web Page or Referrer.
On non-Mkto sites, constrain by Referrer.

You can use the "Filled out Form" filter in a smart list and select the constraint of "Web Page". This will allow those leads to qualify who filled out that form on that specific page.
To view this at the lead level, you can simply look at the activity log and look for "Referral URL":
Are you creating a separate program for each of the form/page combinations? If so, you can do your tracking at the program level.
This is surprisingly easy! There are two ways to do this...
1. Filter - Use a "Filled Out Form" filter and add in a "Web Page" constraint to it. See image below...
2. Trigger - Use a "Fills Out Form" and add in a "Web Page" constraint. See below...
These options can be used in Smart Lists and Smart Campaigns.

Well, the OP doesn't specify different Marketo LPs or different non-Marketo pages, so we don't really know...
Nor do we know if he's looking for some sort of consolidated report with this detail (which really isn't possible within Marketo); or if either of the options that we've listed here address the question.
