Form In Landing Page Showing Up Twice

I see the form code on your page twice as well. Is this a marketo landing page, or CMS?

Because you have the same form twice in the page. Marketo will stack the forms within the same container unless you take special measures as I described here: https://blog.teknkl.com/multiple-forms-multiple-times/
