I've created a Smartlist -- blacklist to keep my competitors, who are posing as prospects, from downloading content. It isn't clear from the documentation -- are people in the blacklist automatically Marketing Suspended?

If the field "Black listed" is checked, yes they will be suspended.
What are the criteria for your smart list ?
You may want to have a smart campaign that sets the "black listed" field to True fot all the members of your "competitor" smart list.

Technically Blackisted and Marketing Suspended are two separate statuses, but they amount to the same thing. Blacklisting is used for you competitors and Marketing Suspended is used for leads that you want to take a break from receiving your emails. This allows you to keep from overwhelming your leads.
Both statuses are completely up to you to assign, and Marketo won't automatically change either.

The other difference is how they handle operational emails. Marketing suspended leads still receive operational emails; blacklisted leads do not.
Black Listing means they will NEVER receive your emails, period. Only use this on competitors, spam traps, and really nasty people. Think of this as the "nuclear" option and do not confuse it with other email fields.
Unsubscribe - can receive operational emails and should be controlled by the lead.
Marketing Suspended - can receive operational emails, controlled by YOU.
Email Invalid - can be controlled by you or the system.
