Exactly. For smart lists you'll be referring to multiple times over, I'd keep them in the Lead Database neatly labeled so their easily found and easy to use. If it's a one-off effort you can either make the smart list in the program or just directly in the smart campaign. -Kim

Yes, please explain the issue(s) you're seeing so we can further trouble shoot. Without knowing anything about the problem I'd just take a random guess that something was edited at the template level without having the emails approved which can cause wacky emails.
