I assumed it had something to do with field tokens, but I wasn't sure if you could "merge" those two fields into one using them. That's awesome, thank you for sharing, Jeff!
Maybe this is due to Marketo not having a Picklist field type? I know that in our CRM the Country value is a picklist, but when I view the field in Marketo the field type is a String field. This may be worth a support case submission to do some further digging into this.

Honestly, I didn't have too much experience working with forms until forms 2.0 came out, so I'm unsure how it worked with the previous version. But after you create the first form 2.0 with the countries added as picklist values, you can clone that form so you don't have to enter in the country value...
Yes, you will have to add those values manually. You can obtain them from your SFDC administrator or view Marketo's list here.
I'd be interested to learn more about how you have these campaigns set up to do this, Jeff. I've run into the same issue where at times I don't want a field to overwrite existing data, but would like to add additional information to it.

Yes. As long as the checkbox isn't already checked on the lead's record, the lead must submit the form for the Contact Me Now for the checkbox value to update/change and for the "fills out form" trigger to pick up the form submission.
