Unsubscribe report with individual records

Can someone please tell me how to create/download a report that will provide the individual unsubscribe records. The email already went out and I have a smart list with the subscribe metrics but I cannot seem to figure out how to download the records/contacts themselves.

Create a smart list:

Unsubscribed=TRUE
Was Sent Email IS "X"

click on Leads and it will show you the list.

If you prefer to see which records unsubbed from just this email:

Unsubscribed from Email IS "X"

should do it.
