This is how my test email shows up on a mobile phone. It's never done this before. I understand that formatting can be different across devices but this seems really abnormal. Can someone point me in the right direction?

I am looking now on iOS device (iPhone 5) and it looks fine to me, aside from a few differences in the underlined hyperlinks.
What mobile device are you looking at the message with? I have other accounts we could have you send to.
Or, perhaps it is an online provider or Gmail?
Although, after further looking at the mobile version, the underline issue is only where it appears that the CSS line-height property may not be adequately large enough to accommodate the underline where other text is just below where the underline should appear.
The mobile version is iPhone 5, but I sent to a hotmail account. Can I change the line height in css and what would that be?
I created a version that places everything in the main body. It still looks the same, I just copied the css into the main body and still having the same issue. I'm no css expert (total novice) but could it be something with the tables used in the template?
Not likely the tables. Will provide further feedback...
