I'm wondering when you all update your most recent campaign/program field. Is it when someone opens/clicks or is it even more broad, for example when an email is delivered or sent?

That's a good question to ask, but I think it is mostly determined by your purpose/use of the field. Not everyone uses this field.
As a best practice I would say that you want to always update your program status. With respect to last program if you are using it to determine engagement then I would associate it to person's actions. If it is to track qualification/frequency the it would be related to an outbound action that represents your attempt to create a touchpoint.
