Hi! Having a strange issue here, we have a checkbox on our form that is checked by default that is not allowing anyone to uncheck.
Has anyone experienced this issue before?
Can you post a link to a page with the form on it for troubleshooting?
Do you mean that the checkbox doesn't uncheck on the page or that when the checkbox is unchecked that it doesn't carry thru to the lead record as "unchecked"?
First of all, edit the Form and make it unchecked, and then approve, and see if it works. If not, then delete it and create a new field with the same logic.
still not solved then need to check it at backend...
It's possible that the unchecked state is not being properly saved or updated in the form's data model.

Suggest we just wait until a link is provided to observe the behavior. No reason to modify the form if the problem (as is most likely) is client side.
